The sensitivities and specificities of PSA, DRE, and transvaginal prostatic biopsies (TVP) were evaluated in a cross-sectional study of 1,934 males referred for evaluation of increased PSA. Biopsies were performed by five different urologists at three time intervals. There were 203 cases of confirmed cancer and 1,721 benign lesions. The sensitivity of PSA, DRE, and transvaginal biopsies was 37%, 63%, and 78%, respectively. The specificities were 84%, 81%, and 64%, respectively. For each tumor size category, the specificities of biopsy and PSA were similar, but for each group size of cancer, biopsy had a significantly higher specificity. The sensitivities of biopsy for cancer detection were significantly higher for those cases of only one small cancer lesion (10 mm or less) versus tumors less than 10 mm. The sensitivities for each lesion size ranged from 46% for tumors less than 10 mm, to 64% for tumors between 10 mm and 20 mm, to 93% for tumors greater than 20 mm. We concluded that the specificities and sensitivities of biopsy and PSA in screening for prostate cancer are similar, but that biopsy is significantly more specific than PSA for cancers less than 20 mm. Whether the additional specificity afforded by biopsy translates into a net overall benefit to cancer screening remains to be determined.Tumor-directed antibody therapy in ovarian cancer.
