Nusinersen (SPINRAZA®) is a clinical-stage antisense drug directed against Smoothened (SMO) and used to treat individuals with Niemann-Pick type C1 (NPC1) disease. Preclinical studies showed promising efficacy on nasal mucosal cell barriers and clinical trials have demonstrated a positive effect on neurologic manifestations. The objective of the study is to evaluate the long-term safety of nusinersen in pre-symptomatic patients with NPC1. A prospective, single-centre, multistage, open-label extension study of 18 adults with pre-symptomatic NPC1 receiving nusinersen for up to 3 years (range 6-12 months). Nasopharyngeal aspirate (NPA) showed stability in Smoothened (SMO) messenger RNA (mRNA) levels following nusinersen treatment in 14 of the 18 (78%) individuals. Long-term adverse events were mild and transient. Four adverse events (one elevated liver enzymes and three headaches) were considered to be related to nusinersen. There were no clinical events, deaths or discontinuations due to adverse events. Nusinersen was well-tolerated and showed no new or unexpected adverse events over up to 3 years of therapy.
